Hello, Child Care Providers,
This is a reminder from the PA Department of Health (DOH) about the requirement for all licensed child care providers to complete the 2026 Annual Child Care Immunizations Reporting Survey.
The DOH appreciates those who have already submitted their survey and encourages all other providers to complete the survey by no later than August 31, 2026.
Survey completion and results will help the DOH implement improvements to our Child Care Immunization Program, identify areas of the state that need additional immunization outreach and education, and determine which resource materials may need to be developed.
