Greetings Child Care Providers,
The PA Department of Health (DOH) is sending out the Annual Child Care Immunization Reporting Survey to all licensed child care providers to meet the immunization reporting requirements outlined in 28 Pa. Code § 27.77 (a)(4) Immunization requirements for children in child care group settings.
The DOH is asking that all licensed child care providers complete and submit the survey at Annual Child Care Immunization Survey 2026 – Take the Survey by August 31, 2026.
The following resource materials have been provided with this email to help with reviewing immunization records and completing the survey:
The DOH developed immunization-tracking worksheets that follow the survey questions. Completing the tracking worksheets first will help child care providers in completing the survey. The tracking worksheets include instructions and are available in a printable (pdf,) and electronic (Excel) version. Only the survey needs to be submitted to the DOH. The tracking worksheets do not need submitted – they are only a tool to help you organize the information needed to complete the survey.
Additionally, DOH and County/Municipal Health Department staff, mostly Registered Nurses, will visit randomly selected child care providers to offer education, resource materials, and other helpful items/activities about the importance of vaccines and to answer any questions. If you are one of the providers that receives a call for a visit, take advantage of the knowledge and expertise these staff have about immunizations and ask for assistance in reviewing immunization records.
