Apply for Keystone STARS Designation Through An Alternative Pathway

Before Completing the Steps Below

In order to apply for Keystone STARS Designation through an Alternate Pathway, you must:

  1. Claim your program’s Organization Profile in the PD Registry. Learn how if you have not already done so.
  2. Provide the most current version of your program’s Continuous Quality Improvement (CQI) Plan. If you complete your CQI Plan outside the PD Registry, you must upload it to the system prior to completing your application. Learn how to access and navigate your organization’s Document Vault.
  3. Review your staff information including their positions and employment dates and update any incorrect entries so your Staff Report is accurate for your Quality Coach. Learn how to verify your staff.

Click Login in the top right corner.

Enter your email address and password, then click Login.

Click on the name of your organization from the top right-hand corner drop-down menu.

Before applying for designation, visit the Program Info tab in your organization’s profile to upload your accreditation. Learn how to upload or edit your organization’s accreditation.

6. To access the Quality Dashboard, you can

a. Select Keystone STARS/Grants tab and click Quality Dashboard underneath or

 

b. Access the Quality Dashboard by selecting Quality Overview from the left-hand menu shown below.

Click Applications.

Click Apply on the section noted as Keystone STARS.

Click on Review for each section marked as review and confirm that information is correct.

Click the Review button for on Pathways and STARS.

Click the appropriate selection whey applying for a STARS designation. Choose Yes if you are seeking designation through an Alternate Pathway. This populates any accreditation you have uploaded to your Program Info tab. Select the accreditation you are choosing as your Alternate Pathway.

Once the required selections are made, select Save & Confirm at the bottom of the screen.

Click the Review Button for on Additional Site Questions.

Answer all questions regarding the site, when finished click Save & Confirm at the bottom of the screen.

Click the Review button for Activities and Initiatives.

Check all applicable options regarding the activities and initiatives at the site and click Save & Confirm at the bottom of the screen when finished.

Click the Review button for Program Yearly Changes.

Answer all questions regarding any changes within the past year and click Save & Confirm at the bottom of the screen when finished.

Complete all sections and denote that they show as confirmed on the screen. Click the Submit Application button at the bottom of the screen when finished.

Review the Terms and Conditions and select I Agree. Click Continue.

Click Yes when you are ready to submit your application.

When you successfully submit your application, you will receive the confirmation below. Click OK
