Directions for completing the Child Outcomes Survey Form - PDF

The Child Outcomes Summary Form (COSF) - Word
The Child Outcome Summary Form, will be used to measure child progress. The results from your child assessments will be used to determine what step on the scale best describes the child’s progress. The use of the 7-point scale will allow for program to program comparisons even though PreK Counts programs are using a variety of assessment tools. This form may be downloaded and printed.

Every PA Pre-K Counts child who is enrolled before December 1 should have a completed COSF that will be reported through the web-based system.

This helpful document provides guidance for decision-making about which step on the 7-point scale best describes each child’s progress. This may be downloaded and printed for teachers’ reference.

The Parent Flyer is a useful resource to use with parents to explain assessment and documentation of children’s progress and the child outcomes reporting process. This flyer may be downloaded and printed for distribution. OCDEL is in the process of printing the glossy version to be sent to PA Pre-K Counts programs for dissemination.
